Client Agreement
Practice information:
We are based within Castle House, which is a large red brick building next door to Banstead Community Hall and close to the Woolpack pub and Banstead High Street. Please enter Castle House using the left-hand side door entrance closest to The Tracery, i.e. please do not use the main front entrance as we are in a self-contained section of the building. Ring the door entry system and walk to the first floor where you will be let in and shown to the waiting room. I will meet you in the waiting room at the time of your appointment.
Parking/Transport:
There is free parking on the residential road nearby Castle House on Yewlands Close. Apologies that we cannot offer parking on site as the spaces are reserved for the tenants of the Castle House. Buses are available in and out of Banstead.
Terms and Conditions:
Sessions are also usually offered in blocks of six and it is recommended that you keep cancellations to a minimum during this time as this can have an impact on the progress you make in therapy.

Also ensure you are aware of the excess due when you make a claim as you will be asked to pay this directly to SurreyPsych.
Cancellations with less than 24 hours’ notice will be charged at the full session rate.
Alternatively, if you are unable to attend your appointment and some notice is given, we will attempt to offer you a replacement telephone or Skype session at a different time during the same week for the usual cost with no additional cancellation fee.
Any notes your therapist writes about your discussions are confidential and will not be disclosed without your consent with a few exceptions.
Exceptions would be where your therapist is required to disclose information by a court of law, or in order to protect your or another from significant harm.
In addition, psychologists are required by their profession to have regular supervision sessions where they may discuss aspects of your therapy. They will not however reveal your identity during these sessions.
